HOME PROVIDERS
As a Home Provider for Community Partners, you are part of a team that works to provide high quality services for individuals who have been identified as needing Residential Services. Together we work to provide the best home environment possible for the people we serve. Home Providers interact with a number of professionals employed by Community Partners and by the State of New Hampshire.
Community Partners Home Providers provide supervision, support, and/or personal care to a person with a developmental disability. Some clients who receive these services may also be identified as having mental health issues.
Home Providers are self-employed and contract with Community Partners. Home Providers are responsible for the client’s day-to-day care, safety, and well-being. They are paid a stipend and a monthly room and board payment, based on the level of the client’s needs.
There are times when Home Providers may enlist the help of others (for example, Home Providers may purchase respite services, seek alternative transportation or ask family or friends to spend time with the client), but the care and well-being of the client remains the responsibility of the contracted Home Provider, and all services, documentation, and other responsibilities must still be met. Individuals providing interim services must also be qualified to provide the service in the same way that the Home Provider is qualified.
During the term of the contract, Community Partners monitors the care of the client and the implementation of the contract. Home Providers are an active participant in the development, implementation, and periodic reviews of the Individualized Service Plan (ISP) for clients in their home. Home Providers provide clients training in daily living skills such as domestic chores, personal care and development, community integration and socialization.
Home Providers are expected to meet the obvious needs of the client by providing food, shelter, transportation, etc., and also support the client in becoming integrated in the community. This provides clients opportunities for interaction in the community to meet their own goals and objectives and to experience being a member of the community in their own right.
