ELDER SERVICES
Our Elder Services staff understand how difficult it can be to ask for help, and are specially trained to help with sensitive issues of older adults related to changes in health, jobs, retirement, family roles, or physical limitations.
We offer a variety of services tailored to meet the needs of older individuals who meet state eligibility criteria:
- Individual counseling
- Psychotherapy groups
- Case management
- Functional support services
- Illness management and recovery
- Supported employment
- Psychiatric evaluations
- Psychiatric consultation and evaluations in nursing homes
- Medication monitoring
- Emergency services
Referral Education Assistance and Prevention (REAP) is also available on a short term basis for elders who are experiencing difficulty in their daily living but do not meet state eligibility criteria. Questions about the REAP program? Email Jennifer Spargo or telephone the REAP Referral Line at 603-516-9325.
