CASE MANAGEMENT

The Developmental Services Case Management program coordinates and assures the continuity of all services and supports for adult consumers (over age 21). Case Management staff also assists in accessing services provided by other agencies. Currently, we are providing Case Management services to over 300 individuals.

Case Management services vary based on the needs of each individual. These needs are identified in the annual Individual Service Agreement (ISA) developed by a team. The team includes the individual, their guardian, other family members, friends and appropriate staff from Community Partners and/or other agencies.

Case Managers are responsible for monitoring that these supports and services are being provided to the individual, and ensuring the quality of these services. Case Managers assist in accessing community activities and resources so that individuals can become active participants in their communities and enhance their quality of life.
