ABOUT NH ADRC at COMMUNITY PARTNERS
The New Hampshire Aging & Disability Resource Center (NH ADRC) is a program of the NH Department of Health and Human Services, who contracts with Community Partners to help individuals in Strafford County access and make connections to:
- Long term services and supports
- Family caregiver information and supports
- Understand and access Medicare and Medicaid options
NH ADRC at Community Partners utilizes the No Wrong Door system model, which means you can rely on us as your single access point to inquire about and obtain long term supports and services in your community. The No Wrong Door network serves anyone seeking long term services and support, regardless of age, income or disability.
Working in conjunction with NHCarePath, NH ADRC at Community Partners helps connect you to a range of information, assistance, caregiver resources and services for aging, disability and independent living, and to counseling and financial planning tools.
NH ADRC at Community Partners provides access to the following services:
- Information Referral and Assistance
- Options Counseling
- NH Family Caregiver Program
- State Health Insurance Assistance Program (SHIP)
- Senior Medicare Patrol (SMP)
At NH ADRC at Community Partners, you can expect to receive courteous customer service and objective and unbiased information on the full range of long term care supports and services and other available community resources. Staff will help to link you with needed services and provide person-centered one-on-one assistance and decision support.
