HEALTH CARE COORDINATION

Help for Children with Chronic Health Conditions & Support for their Families

To be eligible to receive Health Care Coordination services a family must live in Strafford County and have a child age birth to 21 with a chronic health condition, such as:

  • AIDS
  • Cancer
  • Chronic Asthma
  • Crohn’s Disease
  • Cystic Fibrosis
  • Diabetes
  • Dysphagia
  • Epilepsy
  • Muscular Dystrophy
  • Spina Bifida
  • Any Ongoing Illness or Physical Disability

We help families by:

  • Identifying needs
  • Connecting families to respite, resources and services
  • Providing social and recreational opportunities
  • Collaborating with schools, health care providers and other community agencies

The program is administered by the Bureau of Child Centered Services, DHHS NH, in Concord NH. The Bureau of Child Centered Services works in collaboration with all of the statewide NH Health Care Coordination programs to provide NH families with health resources information and support services.

Eligibility Criteria for the NH Health Care Coordination Program

Families in NH may be eligible for NH Health Care Coordination services if they have a child (birth to 21) with a chronic health condition that:

  • Is expected to last 12 months or longer, AND
  • Affects the child’s ability to function daily, and/or
  • Requires frequent and intensive medical care

The Health Care Coordination program eligibility can include clients who have a developmental disability in addition to a chronic medical condition.
