Community Partners Family Support team invites you to attend the 2019 Annual Transition Fair, Saturday September 28th from 9AM - 12PM at the Frisbie Memorial Conference Center in Rochester. A transition fair is an informational event for individuals with disabilities and their families to assist in making a successful transition to adult life and independence.
