As the local community mental health center, we are preparing to become a Certified Community Behavioral Health Clinic (CCBHC). CCBHC’s offer mental health, substance use disorder services and facilitate access to primary care.

Our first step is to conduct a thorough Community Needs Assessment. This is an anonymous survey conducted by an external evaluation team.

Your feedback will provide a better understanding of local behavioral health needs, barriers to care, and opportunities to improve services.
