ProHealth NH is a person-centered health home for young people enrolled in Community Partners Behavioral Health Services. ProHealth NH provides integrated primary care, behavioral health and wellness programs delivered by Goodwin Community Health and Community Partners at the Old Dover Road, Rochester location.
Services Include:
- Evidence Based screening, detection, and treatment of health conditions,
- Evidence Based screening and treatment of behavioral health conditions, including depression, trauma, substance use disorder, need for Assertive Community Treatment, need for Supported Employment and Education, need for help with mental illness symptom management,
- Health programs for tobacco use, fitness, nutrition and obesity that include evidence-based incentives for health behavior change,
- Shared plans of care between mental health and physical health providers,
- Tailoring care to meet the needs of people age 16+, with outreach to engage LGBT and people from cultural and linguistic minority groups,
- Population Management with treatment-to-target goals, and Care Coordination for people with complex needs.
ProHealth NH aims to improve access to care, enhance quality of care and reduce health inequity through Health Home integrated services for 1,100 people age 16+ with SED and SMI by 2024.
